Your Rights & Complaints

As a customer of the ADRC of La Crosse County, you have the right to:

• Be treated with dignity and respect.
• Accept or refuse any information, referrals, or assistance if your own legal decision maker.
• Include friends, family, or others of your choosing in your interactions with the ADRC.
• Inspect your personal ADRC record and to request that it be amended.
• Reasonable accommodations when accessing assistance from the ADRC.
• Not be discriminated against based on race; ethnicity; national origin; religion; sex; age; current or anticipated mental, physical, developmental, or intellectual disability; sexual orientation; genetic information; or source of payment.
• Receive accurate, unbiased information and assistance.

As a customer of the ADRC of La Crosse County, you also have the right to express or file a complaint or grievance regarding the ADRC. We will support customers in the complaint/grievance process and will refrain from any reprisal or threat of reprisal against any individual registering a compliant or grievance.

You may choose to share a complaint or grievance with:

• The ADRC Director/Manager. Contact us by mail, phone, fax, or email.
• The Office for Resource Center Development (ORCD) at the Wisconsin Department of Health Services (DHS). ORCD is the state office responsible for contractual oversight of ADRCs.
