Applicant should be ambulatory, in relatively good physical and mental health and able to tend to their own personal and safety needs. An appointment can be arranged for a tour of apartments and the Carroll Heights facility. Application should be made by completing and submitting the Application Form. The form must be returned to the Housing Manager for review and placement on the wait list. We do a criminal background and credit check on all applicants. A copy of your driver's license will be kept on file and will be asked for at the time of application.
