To make a referral call Family and
Children's intake at (608)785-6054 and state that you'd like to make a
Children with Special Needs referral.
If you are not the parent/legal guardian you must contact the family
and request to make a referral.
Please gather the following information
before calling:
- Child's information (name, address,
city/state, telephone, sex, date of birth, and social security
number)
- Whether or not the child is
receiving medical assistance and the MA number
- Mother and father's information
(address, date of birth, employer, home and work phone numbers)
- Other children in the home and
their date of birth
- If under age 3*, areas of
concern (speech/language, gross motor, fine motor, social
development)
- If over age 3, type of disability
(developmental, physical, autism, medical, mental health)
- Diagnosis of the child
- Name of child's school
- Name of child's doctor and/or
therapist
- Other services the child is
receiving
A referral does not guarantee a child will receive formal services
which may be limited based on funding availability. Some
families may have their needs met through informal supports.
