Youth and Transition



Completing high school is a very important transition time for all students. This is especially true for students with disabilities as they move into adult life. What options do students have after high school? What supports are available? How do students plan for employment, independent living, and postsecondary education? These questions are part of the transition planning process. The ADRC offers a single entry point for information and assistance on issues affecting students and young adults with disabilities regardless of income.

At 17 years, 6 months families should contact the Aging & Disability Resource Center to request a meeting with an Options Counselor that specializes in working with youth transitioning into adult services. Topics that may be discussed include:
  • Transition process from Children to Adult Services
  • Application process for disability benefits such as SSI and Medical Assistance
  • Employment options
  • Post-secondary education options
  • Eligibility determination for publicly funded long term care programs
  • Guardianship or other alternatives
